SUNDRE COMMUNITY VAN
The big Sundre community van can be booked for an individual’s transportation to medical appointments, within Sundre or outside of Sundre, even to Calgary or Red Deer. The van is wheelchair accessible and the cost is $1/km. Call 403-638-3266. On Thursdays the van is operational for the greater community of Sundre. If you need a pick up on a Thursday and you are not a regular user you need to call to have your name put on the list. For other days of the week, especially during busy times. you may need to call one week in advance to book the van. Clients must be able to manage on their own, although a companion may go with them.
There may be drivers available who use their own vehicles. This cost is $0.50/km and the vehicles are not wheelchair accessible. Clients must be able to manage on their own. Please book in advance.

The video was created in partnership between the Gretzky family, Speak Up, the Canadian Society of Palliative Care Physicians, and JMorris Media. You too can decide now and tell your loved ones what you want and make sure they are comfortable with the responsibility. Then write down your wishes, store the document in a safe place and make sure your family knows where to find your Advance Care Plan.
